Home ventilator assessment in Klang Valley: teams, equipment & discharge planning

Already arranging home ventilation or discussing discharge with a clinical team? These are published respiratory assessment routes. They are distinct from an accepted care-home placement, a confirmed nursing roster or approval of a specific post-ICU transfer.

Source pages checked 17 October 2026. This guide supports provider choice; your clinician determines the assessment and treatment plan.

Start with the exact assessment team

Thomson Hospital Kota Damansara’s respiratory service explicitly lists home ventilator care and home oxygen assessment. Sunway Damansara lists Dr Soo Chun Ian with a home mechanical ventilation focus and an individual clinic contact. Ask who will review the discharge plan, equipment requirements, existing records and the care setting. A published team member does not guarantee assignment.

Separate clinical planning from daily staffing

Dr Soo’s clinic describes a chronic respiratory programme including tracheostomy ventilation and home support. That does not establish continuous qualified nurse cover, a residential bed or acceptance of an individual case. Agree who supplies and maintains equipment, provides daily care, trains caregivers, covers nights and responds to changes. Ask the treating and receiving teams to coordinate these responsibilities before discharge.

Compare the complete ongoing arrangement

Request a written plan covering assessment, follow-up, equipment, consumables, staffing, relief, emergency escalation and separate charges. Confirm the actual home catchment and which team accepts responsibility. A live-in caregiver or domestic helper is a different care model; those listings cannot establish ventilator competency or uninterrupted awake nursing.

Questions to take to your shortlisted providers

  • Which respiratory clinician and care team assess home ventilation, and can they review the discharge plan and exact tracheostomy or equipment needs?
  • Who provides and maintains equipment, who is trained for daily care, and what awake staffing, backup power and emergency support need separate arrangements?
  • What hospital-to-home handover, case acceptance, training, follow-up, supplies and ongoing charges must be agreed before discharge?

Keep your enquiry practical

Contact opens the provider’s published channel. You can edit the prepared message before sending. Avoid adding detailed records or identity documents to an initial enquiry; ask the provider how to share them if needed.
